Notes:
This is reference from one of the projects where we had a chance to use the Concerns management process extensively. In our philosophy, a Concern is either a Risk or an Issue. 
The project review MUST deal with both Risks and Issues. 
The Concerns Management system we use is our main tool in creating the review report and makes it very easy both to resolve Concerns and to report them.
